We are happy to share some exciting news about the way we care for you and your health. Our medical practice is now part of a national effort to improve health care across the country. We have been recognized as a Patient-Centered Medical Home, or PCMH for short.

We have worked hard to make changes in our practice to achieve PCMH recognition from the National Committee of Quality Assurance (NCQA). This means our medical practice successfully meets new national standards to provide you with responsive, higher quality, personalized health care. As part of this commitment, we have a dedicated care coordinator at our practice. She will help make sure you get the care you need, at the right time and place.

As part of a PCMH, you will receive:
• More coordinated, personalized health care.
• Comprehensive care that supports you and strengthens your relationship with your doctor.
