Upon your arrival at the office, you will need to present two forms of identification, one of them a PHOTO ID and your current insurance card.

We know that your time is valuable and always make every possible effort to maintain our schedule. Please understand that occasionally emergencies occur which may cause delays.

If you are unable to keep a scheduled appointment, please notify us 24 hours in advance so that we may use that time to care for another patient who may need it.

Referrals
The referral process begins when one of our health care providers or a specialistís office initiates a written request for a referral. Preparation of a referral can take up to 5 business days to complete. Referrals are processed in order of urgency, then by date received. Emergent requests are processed the same day requested.

HMO insurance plans require patients to be evaluated by their primary care physician before seeing a specialist. It is the patientís responsibility to make sure their referrals are up to date and have remaining visits on them before returning to a specialistís office. Keeping track of your referrals will allow you ample time to request additional visits, and provide our office the needed time to process your request.
